Set Up Instruction for Dataform GP Clinic Information System for Windows.

1. Create a new folder, C:\GP Clinic, for this application.
   - if you are UPGRADING, rename the directory before creating another one.
2. Copy or unzip all the files to this newly created folder.
3. Create a file DNS in your "ODBC source administrator".
	- enter "C:\GP Clinic\Clinic_DB.mdb" for the database location
	- enter "GP Clinic" as the DNS name
4. To Test, Click on GP_Prog.exe to start the program.
	- your should have data appearing on the first form

Note: It is assumed that all relevant libraries have been installed. If not, you have to download and run the setup program.
